What Actually Happens in Your Body During an Acupuncture Session?

If you’ve ever had acupuncture, you probably know the feeling.

You’re lying on the table with a handful of tiny needles in place, and somewhere along the way, your body seems to let go. Your breathing slows. Your muscles soften. You may feel heavy, floaty, sleepy—or simply incredibly calm.

But what is actually happening inside your body while you’re lying there?

Acupuncture has been practiced for thousands of years as part of Chinese medicine. Today, modern research is helping us better understand some of the neurological, biochemical, circulatory, and connective-tissue responses that may help explain its effects.

And as it turns out, your body is doing quite a lot while you rest.

When the Needles Go In

An acupuncture needle is incredibly thin, but inserting it into the body creates a meaningful physiological stimulus.

1. Sensory nerve fibers are stimulated

Needle insertion can stimulate sensory nerve fibers, including A-delta and C fibers. These nerves carry sensory information from the tissues toward the spinal cord and central nervous system.

In other words, the needle isn’t simply sitting there. Your nervous system knows it’s there and begins responding to the stimulation.

2. Signals travel to the spinal cord and brain

Once sensory nerves are activated, signals travel through the spinal cord and ultimately influence areas of the brain involved in pain processing, sensation, emotion, and autonomic regulation.

This communication between the treatment site and the central nervous system is one of the important ways researchers believe acupuncture can influence areas of the body far beyond where a needle is placed.

3. Local circulation changes

Needling can also affect microcirculation around an acupuncture point.

Blood flow may increase in the tissues surrounding the needle, bringing oxygen and nutrients into the area and participating in the body’s local response to stimulation.

4. Adenosine increases around the needle

One particularly interesting area of acupuncture research involves adenosine, a naturally occurring substance involved in many processes throughout the body, including pain regulation.

Research suggests that acupuncture needling can increase extracellular adenosine around the treatment site, which may contribute to acupuncture’s local pain-relieving effects.

As Your Body Responds

The response doesn’t stop at the needle site. Acupuncture engages several of the body’s own pain-regulating and signaling systems.

1. Your body’s natural pain-relieving systems are activated

Acupuncture has been associated with the release and regulation of endogenous opioids—pain-relieving substances produced naturally by your own body.

These include chemicals such as endorphins and enkephalins.

Rather than simply “blocking” pain, acupuncture appears to interact with some of the systems your body already uses to regulate how pain is experienced.

2. Pain signals can be modulated

Pain isn’t simply a message traveling from an injured area directly to the brain. The nervous system can amplify, suppress, and otherwise modify those signals along the way.

Acupuncture can engage pain-modulating pathways within the peripheral nervous system, spinal cord, and brain, potentially changing the way pain information is processed.

This is one reason acupuncture is frequently studied for both acute and chronic pain conditions.

3. Neurotransmitter systems are influenced

Research suggests acupuncture can interact with neurotransmitter systems involved in pain, mood, stress, and nervous-system regulation, including pathways involving serotonin and dopamine.

The exact response varies depending on the person, condition, acupuncture points, and type of stimulation used, so it isn’t quite as simple as saying acupuncture universally “boosts serotonin.”

Instead, it appears to influence complex signaling systems the body already uses to maintain regulation.

4. Inflammatory signaling may be influenced

Another growing area of research is acupuncture’s relationship with inflammation.

Studies suggest acupuncture may influence immune and inflammatory signaling pathways under certain circumstances. Researchers are continuing to investigate exactly how these effects occur and how clinically significant they are for different conditions.

Your Nervous System Begins to Settle

This may be the part patients actually notice.

Many people arrive for acupuncture carrying tension they aren’t even consciously aware of—tight shoulders, a clenched jaw, shallow breathing, racing thoughts.

Then, somewhere during treatment, something shifts.

1. Autonomic nervous system activity can change

Your autonomic nervous system helps regulate functions you don’t consciously control, including heart rate, digestion, breathing, blood pressure, and aspects of your stress response.

Research suggests acupuncture can influence autonomic activity, including the balance between the sympathetic “fight or flight” response and parasympathetic “rest and digest” activity.

That may help explain why people sometimes become profoundly relaxed during treatment—or even fall asleep with needles in.

2. Stress-response pathways are influenced

Acupuncture is also being studied for its effects on the hypothalamic-pituitary-adrenal (HPA) axis, one of the body’s major stress-response systems.

Cortisol is part of this system.

Some research has found changes in cortisol and HPA-axis activity following acupuncture, particularly in certain stress-related or disease states. However, it’s more accurate to say that acupuncture may help modulate stress-response pathways than to claim everyone’s cortisol simply drops the moment the needles go in.

3. Your brain is participating, too

Neuroimaging research has observed acupuncture-associated changes in brain regions and networks involved in pain, sensory processing, attention, emotion, and other regulatory functions.

So while an acupuncture treatment may look remarkably uneventful from the outside, your brain and nervous system are actively responding to the sensory input being created.

What About Qi and Blood?

Modern physiology gives us one way to understand acupuncture.

Chinese medicine gives us another.

In Chinese medicine, health depends in part on the appropriate movement and balance of Qi and Blood throughout the body. Acupuncture points are located along interconnected channels, and treatment is designed to influence these relationships.

When Qi or Blood becomes deficient, stagnant, blocked, or otherwise imbalanced, symptoms can develop.

Acupuncture treatment therefore isn’t simply about putting a needle where something hurts.

A practitioner considers the larger pattern: Where is there stagnation? Where is there deficiency? Which channels are involved? What does the person’s overall presentation tell us?

Treatment is then designed to encourage the appropriate movement of Qi and Blood, calm the Shen or spirit when necessary, support areas of deficiency, address areas of excess, and help restore greater balance within the system.

This is also why two people who walk into an acupuncture clinic with the same Western diagnosis may receive very different treatments.

We’re treating the person and their individual pattern—not simply the name of the symptom.

So, Is That Post-Acupuncture Calm “Just Relaxation”?

Not necessarily.

That unmistakable feeling people sometimes experience after acupuncture—the heavy limbs, quiet mind, relaxed muscles, or almost floaty sense of calm—occurs alongside real physiological responses involving the nervous system and other regulatory systems.

And that’s part of what makes acupuncture so fascinating.

Chinese medicine has used acupuncture clinically for thousands of years. Modern research isn’t replacing that traditional framework; it’s giving us another lens through which to investigate what happens when we stimulate the body in this very particular way.

There is still plenty we don’t completely understand about acupuncture, and its effects can’t be reduced to one neurotransmitter, one nerve, or one mechanism.

Instead, research increasingly points toward something much more interesting:

Acupuncture appears to interact with multiple interconnected systems involved in pain, circulation, nervous-system activity, stress, and the body’s own regulatory processes.

So the next time you’re lying quietly on an acupuncture table, remember: There’s a lot happening beneath the stillness.
